EverLife Founder's Member

 

Thank you for choosing to become one of the first members of EverLife Functional Wellness Center. Our Founder's Membership Collection was created to reward the clients who support EverLife during our launch with exclusive introductory pricing, priority scheduling, referral rewards, and additional member benefits available for a limited time.

Please carefully review the following Terms & Benefits before reserving your membership.

 

Membership Eligibility

Founder's Memberships are limited-time introductory offers available exclusively to new EverLife clients.

To qualify:

  • Memberships must be purchased before your first EverLife appointment.

  • Membership enrollment closes September 30, 2026, unless extended or discontinued earlier at EverLife's discretion.

  • Memberships must be reserved according to the requirements of the selected membership level.

  • All qualifying appointments required for your selected membership must be scheduled by September 30, 2026, but may occur anytime through December 31, 2026, subject to appointment availability.

  • All membership appointments must be completed within six (6) months of the original purchase date.

How Your Founder's Membership Works

Reserving your Founder's Membership is a simple two-step process.

Step 1

Complete the Founder's Membership Reservation Form for your selected membership.

Step 2

After submitting your reservation form, you will automatically be redirected to EverLife's secure online booking system to schedule your qualifying appointment(s).

Submitting the reservation form alone does not reserve or activate your Founder's Membership.

 

Your Founder's Membership is confirmed only after:

  • Your qualifying appointment(s) have been successfully scheduled.

  • Your required deposit has been received.

  • Your eligibility requirements have been verified.

Membership benefits become effective only after all required reservation and booking requirements have been completed.

  • 15% OFF your first pre-booked appointment.

  • 10% OFF your second appointment when booked before leaving your first visit.

  • Up to two (2) Founder Friends & Family Referral Rewards (10% OFF each).

  • Automatic EverLife Mailing List enrollment.

Qualification Requirements

  • Your first qualifying appointment must be reserved before September 30, 2026.

  • To receive the additional 10% OFF benefit, your second appointment must be booked before leaving your first appointment.

 
Premier Founder's Membership

Membership Benefits

  • 15% OFF your first two (2) pre-booked appointments.

  • 10% OFF your third appointment when booked before leaving your second visit.

  • Up to three (3) Founder Friends & Family Referral Rewards (10% OFF each).

  • Priority scheduling.

  • Automatic EverLife Mailing List enrollment.

Qualification Requirements

  • Your first two qualifying appointments must be reserved before September 30, 2026.

  • To receive the additional 10% OFF benefit, your third appointment must be booked before leaving your second appointment.

 
Legacy Founder's Membership

Membership Benefits

  • 15% OFF your first three (3) pre-booked appointments.

  • 15% OFF your fourth pre-booked appointment when you book it at your 3rd appointment.

  • Up to four (4) Founder Friends & Family Referral Rewards (10% OFF each).

  • Highest priority scheduling.

  • Automatic EverLife Mailing List enrollment.

Qualification Requirements

  • All 4 qualifying appointments must be reserved at the time of membership purchase.

  • All qualifying appointments must be reserved before September 30, 2026 to qualify for Legacy Founder's Membership pricing.

 

Minimum Service Requirement

Founder's Membership pricing applies to qualifying treatment sessions of 60 minutes or longer, unless a qualifying specialty service specifically states otherwise.

 

Shared Membership Benefits

Every Founder's Membership includes:

  • Exclusive Founder's introductory pricing.

  • Priority appointment scheduling during the launch period.

  • Automatic EverLife Mailing List enrollment.*

  • Personalized treatment sessions tailored to your individual needs.

  • Founder Friends & Family Referral Rewards.

  • Early announcements regarding future services, educational resources, workshops, promotions, and practice updates.

 

Founder Friends & Family Referral Program

Sharing EverLife with your friends and family is one of the greatest compliments you can give us.

Each Founder's Membership includes exclusive Founder Referral Rewards based on your membership level.

 
Discovery Founder's Membership

Up to two (2) Founder Referral Rewards.

 
Premier Founder's Membership

Up to three (3) Founder Referral Rewards.

 
Legacy Founder's Membership

Up to four (4) Founder Referral Rewards.

 

Each qualifying referral receives:

  • 10% OFF one EverLife service.

 

After your referred client successfully completes their first paid appointment, you will receive:

  • 10% OFF one future EverLife service.

 

Founder Referral Rewards remain valid for one (1) year from your membership purchase date.

After your Founder Referral Rewards have been exhausted or expire, you'll automatically transition into EverLife's standard Friends & Family Referral Program.

 

Standard Referral Reward:

  • 5% OFF one future EverLife service for each qualifying new client referral who completes their first paid appointment.

 

Referral rewards:

  • Apply to new EverLife clients only.

  • Are earned only after the referred client completes their first paid appointment.

  • Are non-transferable.

  • Have no cash value.

  • May not be combined with other promotional offers unless specifically stated.

  • One referral reward may be redeemed per appointment.

EverLife Mailing List

All Founder's Members are automatically enrolled in the EverLife Mailing List.

Because Founder's Memberships already include enhanced introductory pricing, the standard 5% Mailing List Welcome Offer is not combined with Founder's Membership discounts.

Mailing List members receive:

  • Practice announcements.

  • Wellness education and self-care resources.

  • Grand Opening updates.

  • Early access to promotions.

  • Future service announcements.

  • Community events and workshop invitations.

 

You may unsubscribe from promotional emails at any time. Unsubscribing will not affect appointment reminders, receipts, required service communications, or your Founder's Membership benefits.

 

Deposits & Scheduling

A 50% non-refundable deposit is required to reserve all qualifying Founder's Membership appointments.

Deposits:

The remaining balance is due at the time services are rendered unless other arrangements have been approved by EverLife.

Appointments are scheduled on a first-come, first-served basis and remain subject to provider availability.

Membership Policies

  • Membership discounts apply only to the original purchaser.

  • Founder's Memberships are non-transferable and may not be assigned, gifted, sold, exchanged, or shared without EverLife's written approval.

  • Services may be upgraded by paying the applicable price difference.

  • Services may not be downgraded after purchase.

  • Promotional offers may not be combined unless specifically stated.

  • Membership discounts cannot be applied retroactively to previously scheduled or completed appointments.

  • Membership benefits have no cash value and cannot be redeemed for cash, refunds, or account credit.

Future Services

The following services will be identified as (Coming Soon After Grand Opening) until officially available:

  • Functional Mobility Programs

  • Corrective Exercise

  • Infrared Therapy

  • Small Group Wellness Training

  • Additional specialty wellness services

 

EverLife's anticipated Grand Opening is October 1, 2026; however, this date is subject to change due to licensing, inspections, construction, equipment delivery, staffing, operational needs, weather, or other unforeseen circumstances.

 

Practice Rights

  • EverLife Functional Wellness Center reserves the right to verify eligibility for all promotional offers.

  • EverLife reserves the right to modify, suspend, discontinue, or substitute future promotional offers, pricing, membership benefits, or eligibility requirements at its sole discretion.

  • Memberships purchased before any future promotional changes will continue to be honored according to the Terms & Benefits in effect on the original purchase date.

 

Acknowledgment 

By submitting your Founder's Membership Reservation Form and continuing to EverLife's online booking system, you acknowledge that you have read, understood, and agree to these Founder's Membership Terms & Benefits, EverLife's Client Policies, Booking Agreement, Cancellation & Rescheduling Policy, Electronic Acknowledgment, and any additional practice policies applicable at the time of service.

By scheduling your qualifying appointment(s), you acknowledge that you understand the membership requirements, booking requirements, deposit requirements, cancellation policies, referral program rules, promotional limitations, and all eligibility requirements associated with your selected Founder's Membership.
