We're honored you've chosen EverLife Functional Wellness Center to be part of your wellness journey.

Your first appointment is designed to help us understand your goals, health history, and individual needs so we can provide the most appropriate treatment for you.

Before your appointment, please review the information below and complete your secure intake forms through MassageBook.

 

 

 

What to Expect

At EverLife, every session begins with a brief consultation to review your health history, discuss your goals, and answer any questions you may have.

​​

Your experience is personalized and may incorporate one or more wellness services appropriate for your individual needs, goals, and the practitioner's education, training, certifications, and legal scope of practice.

​

Throughout your visit, you're encouraged to communicate openly regarding your comfort, preferences, positioning, pressure, or any concerns. Your comfort, safety, privacy, and informed participation remain our highest priorities.

 

 

Before Your Appointment

Please:

✔ Complete your confidential intake forms through MassageBook before your appointment.

✔ Review and electronically acknowledge your informed consent and client policies.

✔ Arrive approximately 10–15 minutes before your first appointment.

✔ Wear comfortable clothing.

✔ Notify EverLife of any recent injuries, surgeries, illnesses, pregnancy, medication changes, or significant health updates before your appointment.

 

 

Scheduling & Cancellation

• To reserve your appointment:

A 50% booking deposit is required.

 

• More than 48 hours before your appointment:

Your booking deposit will be fully refunded.

 

• Between 24 and 48 hours before your appointment:

Your booking deposit is non-refundable but may, at EverLife's sole discretion, be applied one time toward a replacement appointment scheduled at the time of cancellation or rescheduling.

 

• Less than 24 hours before your appointment:

​ Appointments canceled or rescheduled with less than 24 hours' notice, including same-day cancellations and missed appointments, may be charged up to 100% of the scheduled service fee.

 

• Reasonable exceptions:

Exceptions may be considered for illness, family emergencies, severe weather, or other unforeseen circumstances at EverLife's sole discretion.

 

 

During Your Visit

Your appointment is your time.

• Ask questions

• Request more or less pressure

• Decline any technique

• Request modifications for comfort

• Pause or end your session at any time
